 The McMinnville Public Library has access to downloadable and/or reproducible state and federal tax forms. Library staff are happy to help you locate and print tax forms.
Printing costs 5 cents per page.

Please contact the Reference Desk at 503-435-5562 for
which forms we do have available now.

 NOTICE:
We're expecting delivery of most tax forms the
last week of January through the first weeks of Februrary.

 Important Information Regarding State Taxes
Public libraries are no longer a distribution center for state tax forms and booklets. If you filed a paper form last year, you will receive a paper form in the mail this year.
If you need a booklet, call Oregon Department of Revenue at 1.800.356.4222
or e-mail questions.dor@state.or.us.
